Root Cause Analysis is a structured facilitated team process to identify root causes of an event that resulted in an undesired outcome. The process provides a way to identify breakdowns in processes and systems that contributed to the event and how to prevent future events.

Per TEA, the Special Education Self-Assessment is completed annually by LEAs and is intended to engage leadership teams in proactively addressing special education compliance, support student performance, and improve educational programs serving students with disabilities receiving special education services.
